Anatomy

Brachial plexus와 다르게 lower extremity로 가는 신경들은 뭉쳐있지 않다.

lumbar plexus 는 L1~4에서 나오는 anterior rami에 의해 형성되며, T12나 L5에서 나오는 branch도 일부 있다.

sacral plexus 는 posterior cutaneous nerve of the thigh와 sciatic nerve를 낸다. 각각 S1~3에서 나오는 신경 및 L4 and L5에서 나오는 anterior rami가 합쳐져서 생긴다.

Technique

Psoas Compartment Block (Posterior Approach to the Lumbar Plexus)

이 방법은 하지 전체를 마취하기 위해서는 sciatic block과 같이 시행해야 한다. 주요 무릎 및 골반 수술을 받는 환자에서 진통목적으로 종종 시행된다.

1. 환자는 옆으로 누워서 환측 다리를 위로 한다.
2. iliac crest를 잇는 선을 그어서 L4 level을 확인한다.
3. L4 level midline에서 5cm lateral, 3cm caudal에 skin wheal을 한다.
4. 21-gauge, 10-cm stimulating needle을 수직으로 삽입하여 L5 transverse process를 확인한다.
5. transverse process를 약간 cephalad하게 지나게 needle을 통과시킨다.
6. quadriceps motor response를 통해 lumbar plexus를 확인한 후 30 mL of solution 투여한다.

Side Effects and Complications

The approach increases the risk for epidural, subarachnoid, or intravascular injection. Peripheral nerve damage is also a potential risk.
