TURP syndrome (Transurethral resection of prostate syndrome)
Definition
TURP수술 중 사용되는 hypotonic electrolyte-free irrigation fluid가
IV absorption 되어 Symptomatic hyponatremia, excessive intravascular volume, and edema 가 발생하는 것
TURP procedure 의 10% to 15%에서 발생. resection 15분~24시간 내 발생
Risk factor
- 방광내압 증가
- resection시간 증가
- Hypotonic irrigants
- open prostatic sinuses
Signs & Symptoms
- CNS : 두통, N/V, confusion, cerebrla edema, coma
- Cardio-respiratory : bradycardia, hypo/hypertension. pul. edema
Prevention
- Use of conducting (isotonic saline) irrigant with bipolar diathermy
- Monitoring fluid absorption (Input-output 비교) -
- 750mL(F)/1000mL(M) 흡수된 경우 수술 잠시 멈춤
- 1000~1500mL(F)/1000mL(M) 흡수된 경우 수술 중단
- Irrigation시간 제한 (1시간)
- 방광내압은 15~25mmHg이하로 유지 or 70 mm Hg for endometrial procedures
- 부위마취의 경우 환자 신경학적 상태 monitoring
Treatment
- Irrigation 중단
- water restriction
- Loop diuretics
- Hypertonic saline : neurologic Sx 동반된 severe hypoosmolar hyponatremia
