Anatomy

Sciatic nerve (L4,5, S1,2,3)은 piriformis m. 아래를 지나 sacrosciatic foramen을 통과한 다음에 greater trochanter와 ischial tuberosity 사이에 위치함. gluteus maximus 아래 경계에서 제일 superficial 하게됨. 그 다음부터는 thigh 뒤쪽을 지나 popliteal fossa 아래로 진행함.

Indication

saphenous or femoral nerve block과 조합하여 토니켓을 사용하지 않는 knee 아래 부위 수술에 적합함

Technique

Classic (Posterior) Approach of Labat

환자는 옆으로 누워서 환측 다리를 무릎을 굽힌 상태로 앞으로 숙이게 하고, 발 뒤꿈치는 반대쪽 무릎 위에 놓이게 함
PSIS와 greater trochanter of femur 사이에 선을 긋고, 그 선의 중간을 수직으로 통과하는 선을 5cm caudad하게 그음
sacral hiatus와 greater trochanter of femur 사이에 두번째 선을 긋고, 여기에 수직으로 통과하는 선이 needle이 지나가는 부위임
22-gauge, 10- to 12-cm stimulating needle을 삽입하되 bone에 닿으면 좌우로 움직여봄
specific motor end point (e.g., tibial response/foot inversion) 등을 확인하여 올바르게 삽입되면 20~30ml의 solution을 투여함
