Fresh frozen plasma (FFP)
Definition
Contains all plasma proteins, including most clotting factors (except platelets)
Helps prevent inactivation of labile coagulation factors Ⅴ, Ⅷ
(platelet poor plasma -> rapid freezing(-20℃) -> FFP)
Indication
- isolated factor defeciencies ( >1.5 times normal PT or PTT)
- Correction of coagulopathy associated c liver disease
- Urgent reversal of warfarin therapy
- Correction of microvascular bleeding in the presence of increased ( >1.5 times normal PT or PTT)
- -> Received massive blood T/F and continue to bleeding (rarely and only when factors Ⅴ and Ⅷ are less than 25% normal)
- Antithrombin Ⅲ deficiency or thrombotic thrombocytopenic purpura
- Requirements for indications 1 and 6 are prothrombin and partial thromboplastin times at least 1.5 times longer than normal.
* FFP is contraindicated for augmentation of plasma volume or albumin concentration
* For appropriate using FFP : PT > 17 seconds , platelet count > 50,000 cells/mm3 , Blood fibrinogen concentration > 100 mg/dL
Dosage
- To achieve a minimum of 30% of plasma factor concentration (10 to 15 mL/kg of FFP),
- For urgent reversal of warfarin anticoagulation(5 to 8 mL/kg of FFP)
- 1 unit of FFP = Four to five platelet concentrates = 1 unit of single-donor apheresis platelets = 1 unit of whole blood coagulation factors
