Cryoprecipitate
Definition
- High concentrations of factor Ⅷ and fibrinogen
- Contains factor VIII:C (procoagulant activity), factor VIII:vWF (von Willebrand factor), fibrinogen, factor XIII, fibronectin(glycoprotein)
(Platelet poor plasma -> freezing (-70℃) -> slow thawing -> Cryoprecipitate)
Indication
- Hypofibrinogenemia (fibrinogen < 100mg/dL , Thrombin time ↑)
- Treating a uremic bleeding diathesis (unresponsive to DDAVP)
- vWF disease (DDAVP, unavailable plasma derived factor vWF)
*Plasma should not be transfused prophylactically massive red cell T/F or after cardiopulmonary bypass
Dosage
- Per 6unit ; fibrinogen 30-60mg/dL ↑ (If, 70kg adult -> 70mg/dL)
- For uremic bleeding ; 6-10units
- Infusion rate : at least 200mL/hr, using filter, should be completed within 6hours of thawing
