Anatomy

femoral nerve는 L2,3,4 nerve의 post. division에 의해 psoas major muscle 내에서 형성됨.

psoas m의 lat. border에서 나와서 psoas와 iliacus 사이의 groove 통해 내려감, inquinal lig. 밑에서 femoral a.의 옆으로 지나감.

3-in-1 block은 femoral canal 내의 injection이 국소마취제를 lumbar plexus까지 퍼지게 한다는 가정에 기반을 두고 있음. 대개는 옆으로 퍼져서 LFCN과 obturator nerve까지 block하게됨.

Indications

knee arthroscopy as well as analgesia for femoral shaft fractures, anterior cruciate ligament reconstruction, and TKRA

Techinique

환자를 supine position으로 위치. ASIS와 pubic tubercle 사이에 선을 그어 inguinal ligament을 표시함.
femoral artery 위치를 잡은 후 A 22-gauge, 4-cm stimulating needle을 lateral to this line으로 삽입.
대개 anterior branch of the femoral nerve 먼저 확인되며 contraction of the sartorius muscle on the medial aspect of the thigh가 생기게 됨. (이때는 국소마취제를 투여하지 않음)
그 다음에 바늘을 약간 더 lateral로, 더 깊게 삽입하여 posterior branch of the femoral nerve을 찾음.
patellar ascension as the quadriceps contracts가 생기면, 총 20~40 mL의 국소마취제를 투여함. 하지만 obturator nerve block은 30 mL 이상을 투여해도 잘 안될 수 있음.
