Oxytocin (산과마취에서의 사용)

During C/sec

• 태반 만출 후 uterine contraction 을 위해 사용

Dosage

• WHO 권장 : uncomplicated C/sec이후 20 IU를 1L crystalloid에 mix
• Infusion rate
  • 10ml/min (자궁 수축이 적절하고, 출혈이 조절될 때까지.)
  • 이후 1~2 ml/min까지 조절.
• Bolus
  • C/sec시 minimum effective IV bolus dose 는 0.35 IU
  • 0.5~3IU의 small bolus dose에서 uterine tone이 적절해지며, 5IU이상의 bolus 주입시 저혈압 위험성 증가됨.

Complication

• 과량 사용시 자궁 파열이 일어날 수 있음.
• 분만중 과량 사용시 uterine atony도 발생 가능
  • Bimanual massage 후에는 initial treatment와 atony 예방을 위해 oxytocin을 사용.
• 과량 or bolus 주입시 : significant hypotension, tachycardia, nausea, headache
