Definition

: 위 내용물의 역류나 이로 인한 폐 흡인의 위험이 있을 때 사용되는 IV induction의 특별한 방법중의 하나.

Indication

• Full stomach
• 임산부( 2nd trimester이후)
• Aspiration risk 높은 환자.
  • poorly controlled GERD
  • presence of a nasogastric tube
  • morbid obesity
  • diabetic gastroparesis
• Intubation에는 문제가 없으나, mask ventilation이 힘들 것으로 보이는 환자
  • 턱수염, 이빨이 없는 환자 등

Sellick maneuver

• 위내용물이 기도로 역류되는 것을 방지하기 위해 cricoid ring 부위에 압력을 가하여 상부 식도를 막아주는 방법.
• 환자가 깨어있을 떄는 10N, LOC후에는 30N의 힘으로 눌러줌.
• Opponents point of cricoid pr.
  • Lower esophageal sphincter tone을 낮춰 역류를 조장한다.
  • MRI study상 식도가 눌리는 것이 아니라 lateral displacement됨
  • Intubation시 시야에 제한이 생겨 삽관 시간이 늘어나고 aspiration risk 증가할수있음

Methods

• 적절한 preoxygenation후 Cricoid pressure를 가하며 induction
• IV 마취제를 induction dose만큼 빠르게 주입한 후 succinylcholine IV 1~1.5mg/kg주사.
• Positive pressure ventilation없이 ETT 삽입
• 삽입 이후까지 cricoid pr. 유지
