This is a typical sequence of actions required to issue drawings. You particular project may have slightly different requirements. check with the project architect.
CREATE REVISION IN REVIT:

Add button, make it numeric or alphabetic (numeric for prelim, alphabetic for construction).

(Description must refer to package if issued as part of a package.)
To force a revision to appear on a sheet (i.e. there are no revision clouds within views on the sheet):
Place a (very) small bit of Revision cloud on a sheet.
Ctrl Copy it.
Paste > Aligned to Selected Views. Select the views you want to update revisions on.
CHECK TITLE BLOCK & REVISIONS:
Open a Sheet List Schedule (called W_Sheets-for issuing or similar).
You may want to try filter it so only the sheets you are issuing appear (this may not be possible)
Check each revision against a list of previously issued drawings. Make sure the Revit revision is one greater than the previous issue. Fix if not.
Check A_Revision Prefix.SH is set to 0 (zero) if revision is less than 10.
Check Drawn, Checked, Approved are blank or a sensible value.
