Koishi Komeji

The younger sister of Satori Komeji, Koishi, like her sister, is a type of Youkai called a Satori. While Satori typically have the ability to read the minds of others using their third eye, Koishi was unable to deal with the hatred from others who despised this ability, and so she closed her third eye. She succeeded in sealing away her ability to read minds, but she also sealed off her own "self". As a result, she no longer has any conscious thought, and is no longer remembered as a person by those who have met her.

Features:

Closed Eye: Koishi acts according to her impulses, and has no deeper thoughts. Koishi has no thoughts. She is immune to any effect that would alter her behavior, and cannot have her thoughts read.

She cannot be detected by magical means unless the user passes a DC 18 Wisdom saving throw. On a failure, the spell or effect fails to detect her. On a success, the spell or effect works as normal.

A single spell or effect allows the user to make this test only once, regardless of it has a duration, or Koishi moves in and out of its area of effect, etc.

Obfuscate: Just as Koishi does not think of herself, neither do those around her. The weak of mind even forget they ever met her, even seconds after she has left, thinking of her as just a figment of their imagination. Koishi is normally removed from the perceptions of others, and cannot be detected unless a creature actively looks for her.

Koishi can always be seen with truesight.

Whenever a creature attempts to perceive Koishi, it must make a DC 18 Wisdom saving throw. On a failure, it is unable to detect Koishi, and may not make another attempt to detect her until the beginning of that creature's next turn. On a success, it is able to perceive Koishi until the beginning of its next turn.

A creature that Koishi interacts with becomes able to perceive her until the beginning of Koishi's next turn.

Sneak Attack: No-one expects Koishi, and her blows are all the more damaging to those whose minds are unprepared for her attacks. When Koishi makes an attack against a creature under the effects of her Obfuscate, or is otherwise unaware of her, it deals an additional 6d6 damage.

Knife: Melee weapon attack. +9 to hit, 1d4+4 slashing damage.

Reactions:

Behind You!: When Koishi takes damage from a creature, she may immediately cause the attacker to make a DC 18 Wisdom saving throw, or be put under the effects of Obfuscate. If the attacker is within 60 ft. of her, she may then teleport behind it and make a knife attack against it, regardless of whether it passed its saving throw or not.
