Note that deleting the user's specific score/progress won't solve the issue.
Instead, check the user's note page for the exam quiz to see if they have attached an image file. The culprit of the issue could be the content of the notes meddling with the tinymce editor.
Advise the user to only use text when taking notes. Adding images would cause an error.
