CMML
WHO 2022
The prototype and most common MDS/MPN is chronic myelomonocytic leukaemia (CMML), which is characterized by sustained peripheral blood monocytosis and various combinations of somatic mutations involving epigenetic regulation, spliceosome, and signal transduction genes. Diagnostic criteria are revised to include prerequisite and supporting criteria.
Diagnostic criteria (WHO 2022)
Prerequisite criteria
Persistent absolute (≥0.5 10E9/ L) and relative (≥10%) peripheral blood monocytosis.
Blasts constitute <20% of the cells in the peripheral blood and bone marrow.
Not meeting diagnostic criteria of CML or other MPN
Not meeting diagnostic criteria of myeloid/lymphoid neoplasms with tyrosine kinase fusions.
Supporting criteria
Dysplasia involving ≥1 myeloid lineages.
Acquired clonal cytogenetic or molecular abnormality.
Abnormal partitioning of peripheral blood monocyte subsets (voor CMML mn klassieke monocyten, dus CD14+/CD16−).
Requirements for diagnosis
Pre-requisite criteria must be present in all cases.
If monocytosis is ≥ 1 10E9/L: one or more supporting criteria must be met.
If monocytosis is ≥0.5 and <1 10E9/L: supporting criteria 1 and 2 must be met.
Classification
Subtyping criteria
Myelodysplastic CMML (MD-CMML): WBC < 13 10E9/L
Myeloproliferative CMML (MP-CMML): WBC ≥ 13 10E9/L
Subgrouping criteria (based on percentage of blasts and promonocytes)
CMML-1: <5% in peripheral blood and <10% in bone marrow
CMML-2: 5–19% in peripheral blood and 10-19% in bone marrow

The first prerequisite criterion is persistent absolute and relative peripheral blood monocytosis. To enhance diagnostic accuracy when absolute monocytosis is ≥0.5 10E9/L but <1.0 10E9/L, detection of one of more clonal cytogenetic or molecular abnormality and documentation of dysplasia in at least one lineage are required.
Abnormal partitioning of peripheral blood monocyte subsets is introduced as a new supporting criterion. Additional studies are needed to determine the optimal approach to classifying individuals with unexplained clonal monocytosis who do not fit the new diagnostic criteria of CMML.
MP-CMML is commonly associated with activating RAS pathway mutations and adverse clinical outcomes. The blast-based subgroup of CMML-0 (<2%
blasts in blood and <5% blasts in bone marrow) introduced in the previous edition has been eliminated in view of evidence that its addition provides no or limited prognostic significance.
Het klinisch beeld van CMML is uitgesproken heterogeen met een variabele mix tussen
myelodysplastische kenmerken: cytopenie, vaak subtiele dysplasie, ineffectieve erytropoëse
myeloproliferatieve kenmerken: leukocytose, trombocytose, splenomegalie, constitutionele symptomen
extramedullaire lokalisaties komen regelmatig voor: onder meer huid, effusies pleuraal, pericardiaal
auto-immuun of auto-inflammatoire aandoeningen zijn veelvoorkomend (tot 20%): vasculitis (zoals polyarteritis nodosa), artritiden, polychondritis of morbus Sjögren.
Binnen de MDS/MPN overlap syndromen is chronische myelomonocytaire leukemie (CMML) de meest voorkomende aandoening. De incidentie wordt geschat op 0,4 per 100.000 personen per jaar, overeenkomend met circa 75 nieuwe gevallen jaarlijks in Nederland. De ziekte presenteert zich voornamelijk op hogere leeftijd (mediane leeftijd 70 jaar) en bij mannen (66%).

Monocyten
Klassieke monocyten (M1): CD14+CD16-

Niet-klassieke monocyten (M3): CD14-CD16++ (reactief)
Een percentage klassieke monocyten van >94% ondersteunt sterk de diagnose CMML. Andersom kunnen reactieve/auto-immuun oorzaken dit beeld verstoren. Een percentage <94% sluit CMML dus niet uit.
Risico-classificatie
CMML-1-2
CMML-1: 2-4% blasten in perifeer bloed en/of 5-9% blasten in beenmerg
CMML-2: 5-19% blasten in perifeer bloed en/of 10-19% blasten in beenmerg of ≥1 Auerse staaf
Het onderscheid tussen CMML-1, en -2 is geïntroduceerd door de WHO en volgt de afkapgrenzen van MDS met blastentoename. De indeling is prognostisch relevant, hetzij in mindere mate dan geëigende risicoscores. Daarnaast is het van belang voor indicatie van therapeutische opties.
MD-CMML versus MP-CMML
MD-CMML: MDS-type, leucocyten <13*109/l
MP-CMML: MPN-type, leucocyten ≥13*109/l
Het onderscheid tussen het dysplastische en proliferatieve subtype stamt uit de FAB classificatie. Vaak loopt MD-CMML na verloop van tijd over in MP-CMML, vaak geassocieerd met het verwerven van mutaties in signaal transductiegenen (CBL, NRAS, KRAS). Deze indeling blijft bruikbaar omdat de klinische karakteristieken uiteenlopen (MDS versus MPN-lijkend) en daarmee de behandelmogelijkheden.
CMML-specific Prognostic Scoring System molecular (CPSS-mol)
De CPSS-mol score (Elena et al, Blood 2016) combineert klinische gegevens, cytogenetische afwijkingen en mutaties van 4 prognostisch relevante genen en is de standaard bij patiënten die qua fitheid in aanmerking zouden kunnen komen voor een allogene SCT.

Itzykson score
De Itzykson score (Itzykson, Journal of Clinical Oncology 2013) maakt gebruik van 5 parameters, waarbij als genetische input alleen de ASXL1 mutatie gebruikt wordt en geen cytogenetica.

Leucocytose >15*109/l (3 punten)
Hb <6,2 mmol/l (vrouwen) of <6,8 mmol/l (mannen) (2 punten)
Trombocytopenie <100*109/l (2 punten)

Prognose
OS in algemene zin: 30 maanden (aanzienlijke spreiding). Transformatie naar AML: in 20% van de patiënten
Behandeling
Behandelindicatie
In principe wait and see, tot dat er sprake is van progressieve leukocytose, anemie/trombopenie, symptomatische splenomegalie of ziekte-gerelateerde symptomen.
NB. Overige behandelingsmogelijkheden zijn grotendeels palliatief van aard en beïnvloeden het natuurlijk ziektebeloop nauwelijks.

Expert opinions
How I treat CMML (2017): allo SCT, mediane lft 50-56y, OS 33%. Onduidelijk of cytoreductieve therapie voorafgaand aan allo overleving verbeterd. Expert opinion, wel pre-treatment als blasten>10% of hoog-risico ziekte. Keuze uit intensieve chemo of HMA.
Allogeneic hematopoietic stem cell transplantation for MDS and CMML: recommendations from an international expert panel (2017 Blood): Many experts would consider pretreatment with HMA in cases of CMML-2 and an indication for HSCT, preferably early after diagnosis, but evidence from prospective clinical trials and retrospective analyses is lacking.

Uitkomsten:
TRM: 50%, mn agv relapse of GvHD.
Geassocieerd met blastenpercentage: <5% TRM 1y 18% vs >5% TRM 1y 65%.
Geen significante associatie met donor source, type, leeftijd ptn of interval tussen diagnose en SCT.
Relapse rate (5 jaar): 49%, mediaan 4.5mnd na alloSCT.
Overall surival 2y: 21%. Toont daarmee dat een klein percentage patiënten genezen kan worden.

Periode: 1988-2009.
The main objectives of this retrospective study were (i) to analyse non‐relapse mortality (NRM), relapse incidence, relapse‐free survival (RFS) and OS after allo‐SCT in CMML cases registered in the EBMT database and (ii) to identify general prognostic factors.
Periode:
Mediane leeftijd: 53 jaar.
Type donor: sibs (n=285) of MUD (n=228)
Type conditionering: standard (n = 249) of reduced‐intensity (n = 226).
Uitkomsten:
Engraftment: succesvol in 95%
GvHD
Acute GvHD: grade 0–1 in 67%, grade 2–4 in 33%
Chronic GvHD: any grade in 72%, waarvan beperkt in 15% en ernstig in 17%.
TRM
1 y TRM: 31%. 4y TRM: 41%.
Oorzaken: infecties 31%, GvHD 23%, toxiciteit/multiorgaanfalen 19% en overig 27%.
Associaties: TRM was lager bij allo's na 2002 (26% vs 40%) en lager met een kortere interval tussen diagnose en allo (<1 jaar) (29% vs 34%)
Overall survival 4 y: 33%.
Relapse rate 4 y: 32%.
NB. There was no significant difference between CMML‐1, CMML‐2 and sAML. No significant difference in RFS or OS was noted in relation to patient age at transplantation (≤50 years, vs. >50 years), donor‐recipient gender match, HLA‐type of the donor, T‐cell depletion of the graft, administration of TBI with the conditioning).
NB. We were unable to identify any procedure‐related parameters, such as the intensity of the conditioning regimen, the stem‐cell source, T‐cell depletion of the graft, the type of donor etc., with an impact on the transplantation outcome
Hypomethylerende therapie
HMT voorafgaand aan allogene SCT?
Kongtim et al. Biol Blood Marrow Transplant 2016.
Retrospectieve studie, n = 83 patiënten met CMML. Periode 1991-2013.
Mediane leeftijd 57, maar 48% >60 jaar. Type CMML: 47 met CMML-1/2 en 36 met CMML met progressie tot AML.
Type behandeling: 37 HMT (5-aza/deci, tenminste 3x, mediaan 6x), 41 cytotoxische chemotherapie (dus ong 50/50%) en 5 supportive care/hydrea.
Type conditionering: MA in 77%(!) en 23% RIST.
Uitkomst:
Relapse rate (na 3 jaar) was 22% na HMT versus 35% na chemo.
PFS (na 3 jaar) was 43% na HMT versus 27% na chemo
TRM was hetzelfde (27 vs 30%)
Logisch: CMML1-2 doet het beter dan CMML/AML
Doel van HMT: tenminste 3 cycli waarna <5% BM blasten.
NB. in deze studie dus geen vergelijk met GEEN voorbehandeling.

Prospectieve fase 2 studie, n= 8 CMML, 70 MDS (intermed/high risk) en 19 AML.
Interventie: 5-AZA (mediaan 4 cycli, range 1-11) gevolgd door allogene SCT.
Type conditionering: 50% MA en 50% RIST.
Uitkomst:
74% vd ptn gingen door voor allogene SCT (77% MUD)
Daarmee zegt dit stuk dat HMT beter is dan chemo als inductie naar allogene SCT.
NB. in deze studie dus geen vergelijk met GEEN voorbehandeling.
DLI
Results from the Mayo Clinic implied the existence of a graft‐versus‐CMML effect, because the relapse of some patients was controlled with donor lymphocyte infusions (DLI) (Elliott et al, 2006).
