gene expression profile (GEP) related to a GC cell of origin and is enriched for IGH::BCL2 fusion due to t(14;18)(q32;q21) and mutations of genes instrumental for GC development, GC dark zone and light zone transitions and microenvironmental interactions, such as EZH2, GNA13, MEF2B, KMT2D, TNFRSF14, B2M and CREBBP.
Activated B-cell-like (ABC) subtype
derives from cells of GC exit or post GC origin, with either germinal center-exit or early plasmablastic phenotype.
characterized by dependence on BCR signaling and NFκB activities, is negative for most GC markers, and expresses IRF4/MUM1. It is enriched for BCR pathway mutations such as in MYD88 (mostly p.L265P), CD79B and PIM1, as well as genetic changes that block the B-cell differentiation program such as BCL6 rearrangements and PRDM1/BLIMP1 mutation/deletion.
It is recommended to continue rendering the GCB/ABC (GCB/nonGCB) distinction although it has become apparent that the clinical impact of COO stratification is relatively limited outside clinical trials.
WHO 2022: High grade B-cell lymphoma, NOS (HGBL, NOS)
HGBL, NOS represents aggressive mature B-cell lymphomas composed of medium-sized or blastoid cells that do not fit into other well-defined entities. NGS-based analyses of the mutational spectrum and gene expression signatures suggest that HGBL, NOS is a heterogeneous category, also including activated B-cell lymphomas with mutations of MYD88, CD79B, or TBL1XR1. Most frequent mutations are found in KMT2D (43%) and TP53 (30%). By GEP, most cases of HGBL, NOS have been reported to group into the “unclassified” cluster, and the remainder are variably classified in the other clusters. Interestingly, gene expression profiling showed that 54% of HGBL, NOS harbour the “double hit” signature (DHITsig) characteristic of LBCL/HGBL with MYC/BCL2 despite lacking rearrangements of these genes.
WHO 2022: diffuse large B-cell lymphoma/high-grade B-cell lymphoma with MYC and BCL2 rearrangements (DLBCL/HGBL-MYC/BCL2)
DLBCL/HGBL-MYC/BCL2 encompass tumours defined by the presence of dual MYC and BCL2 rearrangements that may be composed of large or intermediate or blastoid cells. This group of cases forms a homogeneous entity with an exclusive GC gene expression profile, a close pathogenetic relationship to FL and molecular GC-like DLBCL subsets. In addition, gene expression signatures associated with DLBCL/HGBL-MYC/BCL2 (such as MHG, DHITsig) significantly overlap with those of Burkitt lymphoma (BL).

morphology similar to Burkitt lymphoma (BL) or with an intermediate or blastoid appearance
immunophenotype (CD10+, BCL6+, BCL2-) and/or gene expression profile (GEP) similar to BL
characteristic chromosome 11q-gain/loss pattern. The losses in 11q24qter are more specific to this entity than the centromeric gains but rarely might be substituted by copy-number neutral losses of heterozygosity. More recent studies have also confirmed that the mutational spectrum, besides the pattern of genomic imbalances, is different from that of BL and is more similar to that of DLBCL of GCB type. Of note, genomic alterations affecting the ID3-TCF3 complex, one of the molecular hallmarks of BL, are only rarely, if at all, seen in HGBL-11q. Cases of B-cell lymphoma with a Burkitt-like appearance that lack MYC rearrangement, therefore, should be tested for the 11q gain/loss pattern. It should be noted that the morphological spectrum of HGBL-11q as defined by the specific 11q-gain/loss pattern is more restricted than that of DLBCL/HGBL-MYC/BCL2.
WHO 2022: Burkitt lymphoma (BL)
The definition of Burkitt lymphoma (BL) in WHO-HAEM5 remains largely unchanged, describing BL as an aggressive mature B-cell neoplasm composed of medium-sized cells with a germinal center B-cell phenotype CD10+, BCL6+, BCL2-/weak, high Ki67 index (>95%) and an IG::MYC juxtaposition.

EBV-negative BL
EBV infection plays an essential role early in pathogenesis causing B cells to evade apoptosis. Emerging evidence suggests a dual mechanism of BL pathogenesis: virus-driven versus mutational, depending on EBV status. EBV-positive and EBV-negative BL share evidence of coding mutations affecting pathways such as BCR and PI3K signaling, apoptosis, SWI/SNF complex and GPCR signaling. In comparison with EBV-negative BL, EBV-positive BL shows significantly higher levels of somatic hypermutation particularly in noncoding sequences close to the transcription start site, harbours fewer driver mutations, particularly in the apoptosis pathway, and shows a lower frequency of mutations in the genes encoding the transcription factor TCF3 or its repressor ID3. To acknowledge these recent insights into BL biology, the distinction of the two subtypes, EBV-positive BL vs. EBV-negative BL, is recommended by WHO-HAEM5.

Dubbel of triple hit
DLBCL met tenminste MYC naast ook een BCL2 en/of BCL6 translocatie. MYC induceert proliferatie maar ook apoptose. Als MYC gecombineerd wordt met een anti-apoptose pathway (BCL2 of BCL6) dan heeft het lymfoom dus vrij baan om te prolifereren.
Diagnostiek
De initiële diagnostiek bij het DLBCL bestaat uit:
Anamnese en lichamelijk onderzoek met speciale aandacht voor orgaanfunctie met het oog op eventuele toxiciteit van de behandeling
Histologisch onderzoek van een lymfklier of betrokken extranodale lokalisatie voor het stellen van de diagnose (WHO-classificatie Swerdlow 2017)
Aanvullend onderzoek ter stadiering en risicoclassificatie (zie Lugano Classificatie)
Bij diagnose: ook FDG-PET/CT scan met diagnostische CT
Risico classificatie via IPI, age-adjusted IPI of R-IPI

Indicatie beenmergonderzoek?
Indien de FDG-PET/CT aanwijzingen laat zien voor beenmergbetrokkenheid, kan beenmergbiopsie achterwege worden gelaten. Indien de FDG-PET/CT geen aanwijzingen laat zien voor beenmerg betrokkenheid, wordt aangeraden een beenmergbiopsie te doen indien dit consequenties heeft voor de prognose en/of het te voeren beleid (zelden het geval).
De sensitiviteit van de FDG-PET/CT voor het detecteren van beenmergbetrokkenheid bij patiënten met een nieuw gediagnosticeerd DLBCL is 89%, de specificiteit 99%. Een positieve FDG-PET/CT leidt in ongeveer 7% van de patiënten tot “upstaging” naar Ann Arbor stadium IV en tot een verandering van behandeling daardoor bij 3% van de patienten. Een negatieve FDG-PET/CT waarbij er wel een positief beenmergbiopt is, leidt zeer zelden of niet tot verandering van stadium of behandeling.

High risk DLBCL (IPI ≥ 3), 21% of patients will relapse within 2-years after completion of R-CHOP induction treatment despite achieving a complete remission. Patient relapsing within a year after R-CHOP treatment have a very poor prognosis, even after second line chemotherapy, with only 15% of patients achieving a long remission. Therefore, additional therapy in first line treatment is required for these patients. The immune checkpoint inhibitor atezolizumab is a monoclonal antibody directed against the program death ligand 1 (PDL1).
Double-hit or triple-hit high-grade B cell lymphoma have a dismal prognosis following standard treatment with R-CHOP. Intensified treatment with DA-EPOCH-R induces higher complete metabolic remission (CMR) rates and prolongs disease free survival (DFS) as compared to R-CHOP and other intensified chemotherapy regimens, but relapses do often occur and improved overall survival (OS) has not been achieved. Check-point inhibitors like nivolumab show promising results in relapsed NHL.

Achtergrond: fase III FLYER studie heeft laten zien dat bij patiënten met aaIPI 0, < 60 jaar (zonder bulk) 4xR-CHOP21+2R even effectief en minder toxisch is dan 6x R-CHOP21. De keuze van therapie is ook afhankelijk van de verwachte toxiciteit van de radiotherapie (lokalisatie, grootte veld) versus de verwachte toxiciteit van antracyclines.
Waarom 2 extra kuren rituximab? Er is geen toegevoegde waarde van 2 extra kuren rituximab na 6x R-CHOP bij een negatieve PET interim(19), maar aangezien interim PET niet geadviseerd wordt buiten studieverband wordt voorlopig na 6 kuren nog wel geadviseerd 2 keer rituximab te geven bij patiënten met stadium II (niet beperkt)-IV .

Indicatie voor CZS profylaxe?
In het algemeen is het risico op CZS lokalisatie van een DLBCL laag (2-4%) zeker na behandeling met Rituximab. Indien verschijnselen van een CZS recidief lokalisatie optreden, dan is dit meestal een vroeg verschijnsel, mediaan na 5-8 mnd dus waarschijnlijk al eerder subklinisch aanwezig. Dit geldt mogelijk nog meer na behandeling met rituximab. Dit ondersteunt vroeg diagnostiek eerder dan profylaxe. CZS recidief heeft een slechte prognose, met 1 jaars overleving 22-58% na transplantatie.
Risicofactoren voor het optreden van een CZS recidief: de meeste publicaties betreffen de periode voor rituximab en zijn kleinschalig geweest. Daarbij zijn klassieke risico-lokalisaties zoals testis, één of meer extranodale lokalisaties, hogere leeftijd, beenmergbetrokkenheid en hoog LDH vaak genoemd. Enkele meer recente publicaties laten een veranderd risicoprofiel zien waarbij sommige vroegere bekende risicofactoren zoals sinus en orbita niet meer afzonderlijk van belang lijken te zijn.
Het prognostische CNS-IPI model van Schmitz lijkt effectief en bruikbaar alleen zijn patiënten met HIV noch moleculaire factoren hierin meegenomen. Moleculaire factoren lijken eveneens een belangrijke prognostische factor te zijn, mn MYC+ en double hit lymfomen hebben een verhoogd risico op CZS lokalisatie.
Advies:
CNS-IPI < 3 = kans op CZS recidief <5%. Dus geen LP of profylaxe
CNS-IPI 3 + epidurale lokalisatie, of CNS-IPI 4 = kans op CZS recidief 5-10%. Dus LP aanbevolen (dwz liquor onderzoek op celgetal, cytologie en immunodiagnostiek). Indien LP negatief geen profylaxe. Indien LP positief, dan uiteraard behandelen.
CNS-IPI 5-6 = CZS risico >10%. Dus start profylaxe.
Profylaxe wordt geadviseerd indien CZS risico > 10%. Dus ook bij
DLBCL lokalisatie in testis/ovarium/nier/bijnier

Follow up na 1e lijns behandeling DLBCL
Na de behandeling van DLBCL-patiënt adviseert de werkgroep controle in eerste jaar 3-maandelijks en in het tweede jaar 6-maandelijks. Er is geen bewijs voor het maken van CT-scans dan wel PET-CT gedurende follow-up. Recente literatuur en de ESMO richtlijn suggereert dat 2 jaar follow-up voldoende is gezien de kleine kans op recidief na het tweede jaar.(Sort B)
In geval van radiotherapie adviseert de werkgroep om de follow-up te verlengen tot levenslang indien er sprake is van grote velden waarin vitale organen zoals het hart, longen, grote vaten en mammae. De follow-up kan dan volgens het ‘BETER” protocol worden verricht en begint 5 jaar na de behandeling.

Patiënten met een recidief of refractaire ziekte van het DLBCL hebben een prognose van enkele maanden zonder behandeling. Patiënten met primair refractaire ziekte hebben een mediane overleving van 6.3 maanden indien tweede lijnstherapie wel wordt gestart. Patiënten met recidief DLBCL en tijdens 2e lijns behandeling chemosensitieve ziekte hebben ziekte vrije overleving van 30-60%.
Te overwegen regimes re-inductie therapie: Er zijn drie gerandomiseerde studies verricht, R-DHAP versus R-ICE en R-DHAP versus R-GDP, R-DHAP versus O-DHAP.
Re inductie therapie met R-ICE en R DHAP geven dezelfde uitkomst ORR 63%, CR 38%, OS 50% (mogelijk is R-DHAP beter voor GCB lymfomen)
Re inductie therapie met R-GDP en R-DHAP = gelijke uitkomsten (R-GDP is minder toxisch en het kan poliklinisch worden gegeven)
Re inductie met ofatumumab -DHAP is niet beter dan Rituximab -DHAP.
Advies:
Fitte patiënt, geschikt voor autologe SCT (vaak <70 jaar): with auto-HCT, a 5-year EFS of 46% and 5-year OS of 53% was seen

Allogeneic hematopoietic cell transplantation for diffuse large B cell lymphoma: who, when and how?
Despite overall improvements in outcomes of patients with DLBCL, ∼30-40% of patients develop relapsed or refractory disease. For patients with chemo refractory disease, or recurrent disease following autologous hematopoietic SCT (auto-HCT), the prognosis is poor, with no consensus on the optimal therapy. In many cases, allo-HCT applicability is limited by patient age, comorbidities, performance status and treatment-related toxicities. Recent attempts to improve the efficacy of auto-HCT, such as incorporating radio-immunotherapy into the conditioning regimen, have not resulted in improved outcomes.
Respons: Allo-HCT in relapsed/refractory DLBCL patients can result in a 30-40% PFS rate at 3 years, in part due to a graft vs DLBCL effect.
Conditionering:
In a recent CIBMTR report, we analyzed the outcomes of 396 adults who received allo-HCT for DLBCL primarily from unrelated donors (67%), following myeloablative conditioning (MAC; n=165), RIC (n=143) or NMAC (n=88) between 2000 and 2009.
RIC and NMAC recipients were older, more likely to have received prior auto-HCT, prior irradiation and more prior chemotherapy regimens than MAC recipients.
The non-relapse mortality (NRM) rate at 5 years was significantly higher in MAC recipients than in the RIC and NMAC groups (56% vs 47% vs 36%; P=0.007).
Lymphoma relapse/progression at 5 years was significantly lower in the MAC group than in the RIC and NMAC (26% vs 38% vs 40%; P=0.031) groups, but the respective 5-year PFS (18, 15 and 25%) and OS rates (18, 20 and 26%) did not differ significantly.
These results were in close agreement with a previous study from van Kampen et al., who reported on the EBMT experience in 101 patients with relapsed/refractory DLBCL, receiving either MAC (TBI/CY or BU/CY) or RIC allo-HCT (mostly fludarabine-based) following a prior auto-HCT.
The 3-year NRM was 41% after MAC allo-HCT, compared with 20% after RIC allo-HCT.
The 3-year PFS and OS were similar for MAC and RIC patients at 42 and 52%, respectively
Thus, MAC has been associated with increased NRM and (in some reports) decreased relapse incidence, compared with RIC.
Therefore, the collective results of several, mostly retrospective, studies indicate an increased NRM and decreased relapse incidence after the use of MAC regimens. Although MAC allo-HCT seems to have no clear OS advantage over RIC, in general, for patients with relapsed and refractory DLBCL, for the subgroup of DLBCL patients who are young and fit, with high-risk disease, it is logical (although not yet proven) that MAC may afford better disease control vs RIC, without as much increase in NRM risk. For older DLBCL patients, those with comorbid conditions, or those with prior auto-HCT, RIC or NMAC appears to be the better option, particularly for patients in a CR prior to allo-HCT. For younger, fit patients with high-risk disease who achieve a CR prior to allo-HCT, RIC may also be preferred owing to a lower NRM than MAC, and reduced need for immediate antitumor effects compared with patients with active disease going into transplant.
