Cold agglutinin disease (CAD) is an autoimmune haemolytic anemia mediated by monoclonal cold agglutinins and driven by an underlying clonal B-cell lymphoid proliferation not fulfilling criteria for a B-cell lymphoma. The annual incidence of this rare disease is estimated at 1–1.8 per million; its prevalence is four-fold higher in colder countries.
Algemene behandeling AIHA
Niet transfunderen tenzij (Hb=3.5 mmol/l, symptomatisch of acute bloeding) vanwege verhoogd risico op additionele allo-antistoffen

Voorts ruim hydreren, eventueel alkaliniseren.
Parvo B19 veilig (indien B19 serologie negatief is)

Drie categoriën koude-gevoelige antistoffen
Koude agglutinines: antilichamen die antigenen op RBC herkennen bij koude. Veroorzaken RBC agglutinatie en extravasculaire hemolyse, m.a.g. anemie en vaak geen hemoglobinurie.
Donath-Landsteiner: antilichamen die RBC antigenen herkennen bij koude, maar in tegenstelling tot koude agglutinines, binden deze antilichamen comlement en veroorzaken intravasculaire hemolyse (kliniek: hemoglobinemie en hemoglobinurie (flank pijn, donkere urine), ook wel genoemd: paroxysmale koude hemoglobinurie (PCH). Vaak IgG en gericht tegen "P" antigeen op RBCs.
Cryoglobulines: antilichamen die immuuncomplexen vormen bij koude. Reageren vaak niet op RBC en geven dus geen hemolyse, maar veroorzaken systemische vasculitis, inflammatoir syndroom of vasculaire occlusie.
Koude agglutinatie ziekte is een vorm van AIHA. Naamgeving op basis van kliniek (klachten geluxeerd door koude) alsmede labfenomeen (agglutinatie in vitro bij 4°C).
In de lichaamskern binden de IgM-agglutinines niet aan de RBC, maar in de periferie koelt het bloed af en kan IgM binden aan de RBC. Deze IgM-RBC binding activeert de complement-cascade (C3b). Een C3b gecoated RBC stroomt terug naar de kern waarbij in warmte het IgM weer dissocieert. C3b-gecoated cellen worden vervolgens opgeruimd in de lever, resulterend in extravasculaire hemolyse (soms ook intravasculair). De ernst van hemolyse is afhankelijk van de "thermal amplitude" en in mindere mate van de IgM serum concentratie.
Eigenschappen koude agglutines
Isotype: meestal IgM. IgM is een pentameer, waardoor ook een laag aantal antilichamen al overte agglutinatie kan veroorzaken. Lichte ketens: meestal kappa.
CAD in 90% of patients is an IgM–mediated process, with rare findings of monoclonal IgG, IgA, or λ light chain restriction, whereas warm AIHA is predominantly an IgG-driven disease.
Specificiteit: meestal anti-I or anti-i
There are a variety of antigenic epitopes on the surface of RBCs, many of which have been serologically defined as blood group antigens. Cold agglutinins react with RBC antigens that contain polysaccharide (carbohydrate or sugar) epitopes on glycoproteins or glycolipids such as ceramide or glycophorins. The vast majority of cold agglutinins are directed against "I" or "i" antigens of the I blood group (referred to as "big I" or "little i" respectively). The big I and little i antigens are ubiquitous (present on all cell membranes) and structurally related. Little i is the predominant structure on fetal RBCs. It consists of a straight-chain polymer of at least two lactosamines. After birth, an enzyme becomes activated that can add additional lactosamine disaccharides to galactose, resulting in a branched chain structure that constitutes the big I antigen. Thus, little i is progressively converted to big I during infancy, and most children become big I positive by the age of two years. The function of the big I and little i antigens is unknown.
Titer: the titer is the number of dilutions after which the antibody can still cause agglutination; it reflects antibody concentration and avidity. Generally, a titer ≥64 is considered clinically significant. The titer level is less concordant with disease activity because hemolysis occurs with levels as low as <1:32 ; however, most consider titer levels greater than 1:512 as clinically significant.
Thermal amplitude: cold agglutinins react with RBCs at colder temperatures than normal core body temperature. The temperature range at which they are active is referred to as the thermal range or thermal amplitude. The typical thermal amplitude is in the range of 3° to 4° C (ie, refrigerator temperature), but many antibodies are active at higher temperatures such as occur in colder, acral areas of the body
Klonaliteit: meestal monoklonaal.
Monoclonal cold agglutinins are more common; they are typically associated with lymphoproliferative disorders; and they are more pathogenic than polyclonal cold agglutinins. Derived from a single clone of lymphocytes; having a single, uniform epitope specificity. Monoclonal IgM CAD is classically seen in older adults; it is a long-term disease that often resists treatment and may be associated with an underlying lymphoproliferative disorder. Some evidence suggests clonal light chain dominance in 90% of patients with CAD. Cytogenetic studies have noted trisomies 3 and 12 and the t(8;22) mutation.
Polyclonal derived from multiple independent lymphocytes; having multiple epitope specificities. Polyclonal antibodies, typically seen in the postinfectious setting, are self-resolving, although treatment of the underlying infection may hasten resolution. Postinfectious cold agglutinins are seen with viral and bacterial pathogens, including mycoplasma, Epstein-Barr virus and legionella. Case reports of cold agglutinin hemolysis induced by varicella, Citrobacter, and influenza have been described. Polyclonal IgM antibodies are most commonly seen in children.
Kliniek
Koude agglutinatie ziekte treedt meestal op rond 70 jaar, gemiddeld iets vaker vrouwen.
Anamnese/LO: livedo reticularis, Raynaud, acrocyanose en zeldzamer cutane necrose. Minder vaak splenomegalie en hemoglobinurie.
Lab: hemolyse, positieve Coombs (anti-C3 POS)
Geassocieerde ziektebeelden: in 70% geassocieerd met lymfoproliferatieve ziekte (NHL), maligniteit (sarcoom, carcinoom, melanoom), infectie of autoimmuun ziekte (lupus, RA).
Differentiaal diagnose CAD: IgM monoclonal gammopathy of undetermined significance (MGUS), Waldenström macroglobulinemia.
Behandeling
Koude vermijden
Indien ernstige anemie, overweeg transfusie (NB. Cold agglutinin antibodies complicate crossmatch because agglutination causes difficulty in detecting blood type and alloantibodies)
Medicamenteuze overwegingen:
Doel: onderdrukken aberrante IgM productie
Rituximab 375 mg/m2 gedurende 4 weeks (response rate 45-58%)
Alternatief: corticosteroids (response rate 14%), alkylerende therapie en purine nucleoside analogen. Eculizumab (meer ervaring bij PNH)
NB splenectomie niet zinvol (daar vindt de hemolyse niet plaats)
AIHA door medicatie
Medicatie kan in een positieve DAT resulteren, met soms hemolyse tot gevolg.
