Distance acuity is measured by:
Client sits 3 metres in front of the vision chart.
Ask the client to cover one eye and read the smallest line they are able to read with ease.
If they miss several letters ask them to try the next line up. If they only miss one have them repeat the line. If they continue to have difficulty have them try the next line up.
Repeat on the other eye, reading in the opposite direction.
Then do both eyes open.
Vision is tested both uncorrected and corrected.
Interpretation:
6/6 = normal vision
6/12 = reduced vision (Australian legal driving limit)
6/18 = low vision
6/60 = legal blindness
Near vision is measured by:
Hold the near vision chart (Sussex) 30cm in front of client's face.
Ask the client to cover one eye and read the smallest line they are able to read with ease.
If they miss several words ask them to try the next larger font. If they only miss one word have them repeat the line. If they continue to have difficulty have them try the next larger font.
Repeat with the other eye.
Then do both eyes open.
Vision is tested both uncorrected and corrected.
Interpretation
N6 = newsprint
N12 = common print for daily life
N16-18 = large print
Procedure
Sitting directly in front of the client, ask the client to focus on your ear, maintaining a a straight-ahead gaze.
Ask the client to cover one eye and say "yes" when they first see the target appear (i.e. pen or fingertip).
Assess far right, far left, lower and upper directions.
Repeat on the other eye.
Procedure:

Utilise the Farnsworth D-15 bookmark on the Medical computer.
Seat client in front of computer and request they arrange in order of previous hue.
Document findings.

Ask client to hold index finger 30cm in front of nose and focus on fingertip. Move finger to touch nose. Both eyes should centre and evenly appear cross-eyed.
Ask client to look straight ahead/looking at practitioner. Practitioner to cover one of client’s eyes – opposite eye should remain straight ahead.
Repeat with opposite eye.
