A referral will be made by a Water Corp representative. Included in the email referral should be their job description/key tasks analysis and the report template which is attached.

Following your assessment please:

• Complete the medical certificate of capacity.
• Provide certificate of capacity and email of the referral to reception for scanning and attachment to Nookal
• Reception will then be able to be emailed to the Water Corp
