EVOLUTION WORK INJURIES

Evolution will periodically refer injured workers to physiotherapy for management of injuries sustained on site. The procedure is as follows:

• • review injured worker

  • provide appropriate treatment

  • complete report as per Evolution report template on Nookal

  • ask reception to email report to Evolution

The Evolution treatment report is located in the letter section of Nookal. Complete all areas of the form and ensure it is emailed off following completion.
