Select “New Pre-Employment Paperwork File”
Within the folder there are folders with the names of all the business who we do medicals for
Select the desired folder
Print the paperwork
When you organise the paperwork, the order should be:
§ Patient Details form
§ Questionnaire
§ Pem Completion Checklist
§ Clinipath COC DAS form/ Instant DAS form (if required)
§ Audio form 18 and 406 (if required)
§ Medical Paperwork
§ M+F/Physical
NOTE: the paperwork varies depending on which company is requesting the medical and what components are required
Use a paperclip to hold all the pages together
Put a sticky note around the pages the candidate is required to complete
§ Make sure the sticky part of the sticky note is stuck to the back of the last page the candidate is required to complete and to the front of the first page they are required to complete. This prevents any pages slipping from between the sticky note and prevents the candidate from missing pages
On the sticky note write the details of the appointment, which are as below:
§ For letter of the candidates first name and Last name (ie J. Doe)
§ Day and date of appointment (ie Tue 14/09)
§ Time of appointment (ie @10.00am)
§ What company has requested the assessment (ie Knight Health or KH for short)
Place into relevant dated Pigeonhole
§ If the paperwork is required for the current day place it on a clipboard in relevant time order
