Medical Questionnaire

Each candidate will fill out a medical questionnaire. It is important to review this document and seek further clarification and make notes on any positive responses. In general you should ascertain the following:

• • when the injury / condition first occurred

  • how it was managed or treated

  • if it is requiring continued management or treatment

  • if there are any restrictions in relation to the injury/condition

  • if the injury / condition is fully resolved, note the last episode or occurrence

If the candidate has respond on the questionnaire that they are on medication please ascertain:

• • how long they have been taking medication

  • for what purpose they take the medication

  • if they are taking the medication regularly (this is very important in the case of blood pressure medication)

  • if possible obtain a photocopy of current prescription or label on the medication box
