To access Lead Enquiry Form:
Log into DilogR: http://dilogr.com/app/login

Click on Survey
Click on the project LOA Coaching Enquiry Form
To access Questionnaire Form:
Log into DilogR
Click on Survey
Click on the project LOA Coaching Enquiry Form Questionnaire
To edit forms:
To edit questions:
On the right tool bar under Create section click on Questions
Make any changes to existing questions by clicking on the name of the question, editing and clicking Save
Close the window by clicking X on the top right
Delete questions you don’t need
To add new questions click on Add New Question at the bottom of the question list
To edit logo and headline:
On the right tool bar under Create section click on Headline/Subhead
Edit the Headline (or leave it empty)
On the right tool bar under Create section click on Preview and make sure everything looks good
If you need to generate PDF reports for this survey:
On the right tool bar under Create section click on Recommendation
Click on Individual Answers on the top
Under EACH question click on Create Recommendation button
Make sure that in the field next to Recommendation intro text, everyone who answers this question sees this (regardless of answer): there is the correct name of the question, for example:

Make sure that in the field next to Recommendation if they answer this question there is the correct recommendation of the question, for example:

Click Save
On the right tool bar under Customize section click on Email Settings and make sure the following fields are correct:
From Email: info@inspiredspiritcoachingacademy.com (or other email address)
From Name: Inspired Spirit Coaching Academy (or other name)
Subject: Put Your Subject Here
Bcc: sandy@wildlywealthy.com, anastasiabarre@gmail.com and your email address

Make sure Include a downloadable report in the email is checked
Make sure Download as PDF is chosen
Make sure Report Intro Text is:

Click Save
To edit lead capture form:
On the right tool bar under Customize section click on Lead Capture and make sure the following fields are checked:
Email Address Required
First Name Required
Last Name Required
At the bottom make sure to check Hide opt-in form if form fields are present in URL Paramater
Click Save
To edit thank you page:
On the right tool bar under Customize section click on Thank You Page and scroll down to Thank You Page Behaviour.
Update the link for thank you page.
If you need to integrate with Infusionsoft:
On the right tool bar under Integrate section click on Infusionsoft and check Enable Assessment Tagging and choose the appropriate tag
To get the code for the questionnaire and update on the site (IMPORTANT: if you make changes to the questionnaire, it will automatically update the embed code on the website. HOWEVER, if you do any changes to LEAD FORM, you have to reembed the new code on the WP page)
On the right tool bar under Distribute section click on Embed
Click on Simple Iframe Tab
Under Survey Design choose Pages
Adjust Survey Height so you can see your survey all the way to the bottom (you will have to do this after first testing)
Check Use secure navigation(https)
Copy the code
Log into ISCA WP website and find the page for the questionnaire
Open the HTML code editor
Insert the code from DilogR
Inside the code find scrolling and change it to no
Save and test the page and the survey.
Done.
