Log into google forms (from GSuite dashboard): https://docs.google.com/forms/u/0/?tgif=d
Open "XXXXX Medical & Waiver Form"
Click on three dots on top right hand side of browser
Click "Make a copy"
Name the file
Choose the new client folder created in Program Delivery and press "Ok" to save the file.

When in the form, click on "Send"
Next to "Send via" select the "paperclip" option and make sure you select the tick box to "Shorten link"
Select "Copy" and then paste this copied link into an email

In Forms, open a form or quiz.
In the panel above the form, click the Responses tab.
Click More. Get email notifications for new responses.
