How to Make Google Docs as Fillable Form

Step 1: Prepare a template document in Google workspace - Google Docs

Step 2: Create a notation with placeholders in "${xxx}" format for fields that you want to fill 

Step 3: Click Add-ons in the toolbar > Get Add-ons

Step 4: Install the  "Fillable Document" add-on

Step 5: Choose an account to sign in with Google

Step 6: Allow all permissions. The add-on will now be available for use

Step 7: Click Start to use "Fillable Document" add-on

Step 8: The add-on automatically identifies all the dynamic fields and list them

Step 9: Click Start Setup > Choose Automatic Setup or Manual Setup

Step 11: Check the fields. Edit the field if necessary by clicking on the pink icon before the field name (e.g. required field, type of field, date format, etc.)

Step 12: Launch the fillable form. Click Open Web Form

Step 13: Fill out the form details

Step 14: Once the form is filled, Save the form. This gets saved in the destination folder which was set up automatically

Step 15: Click Generate the document. This will generate new PDF and DOC files with the merged information

Step 16: Download the document or send the document as email
