Mission: To increase and strengthen collaboration among all sectors of the Fond du Lac County to improve the emotional, mental and physical health of communities members by providing early identification of mental health disorders, risks of suicide and substance abuse.

Clientele: CSI aids clientele throughout the lifespan

Services: Comprehensive Service Integration (CSI) is a county wide collaborative initiative between community partners including law enforcement agencies, the Department of Community Programs, Department of Social Services, Fond du Lac and North Fond du Lac School District and other community stakeholders. CSI encompasses four element units which aid in supporting our county which include, safe school and violence prevention, drug free communities, mental health access (such as YScreen, prevent suicide, destination zero, and trauma informed care), and SPROUT.

Internship Description (120 Hours): Position Title: YScreen, Destination Zero, CSI program intern reports to YScreen program coordinator; Destination Zero coordinator; CSI Coordinator.

QUALIFICATIONS:

• A highly motivated and organized individual who exhibits leadership skills and the desire to work with youth and adults regarding prevention and early identification in the areas of substance use/abuse and mental health.

• Willingness to collect and enter data and assist in implementation of strategies and activities.

• Excellent verbal and written communication skills with all ages and venues.

ESSENTIAL JOB FUNCTIONS:

1. Collaborate with Program Coordinators to implement comprehensive approaches to prevention, early identification and intervention strategies in the areas of substance use/abuse and mental health among middle and high school students (YScreen) and throughout the lifespan (Destination Zero/CSI).

2. Become knowledgeable about the issues surrounding mental health as it pertains to communities as a whole.

3. Become knowledgeable about adolescent mental health screening and best practices in early identification and connection to services.

4. Work with coalition members to collect and review assessment data as related to implementation and evaluation of objectives and strategies.

5. Assist with the development, implementation and delivery of services related to mental health screening within the school district, Fond Du Lac and surrounding communities.

6. Assist with social media maintenance for YScreen, Destination Zero, and CSI.

7. Increase communication and collaboration with county-wide initiatives.

8. Work with community on implementation of evidenced-based strategies.
