Mission: The mission of Agape of Appleton, Inc. is to provide quality residential support services for individuals in a variety of community settings.
Clientele: We serve individuals with disabilities living in our 24 hour programs:
-Community Based Residential Facility (CBRF)
-Adult Family Homes (AFH)
-Supported Apartment Living
-Level 5 Foster Home
Services: Provide participants with ongoing instructions in self-care and daily living skills.
Encourage and support participants with their social relationships and in their planning and use of their leisure time.
Assist participants with the upkeep of their personal living quarters.
Transport participants with agency vehicles or personal vehicle.
Transfer and lift participants according to their needs, using proper techniques to ensure the safety of all involved.
Be capable, both mentally and physically, to assist participants during emergency situations such as, but not limited to: evacuation of premises, physical aggressions, and assistance during epileptic seizures.
Monitor participant's behavior and conduct, encouraging and supporting appropriate behavior and socialization with others, and appropriately handle challenging behaviors.
Internship Description (120 Hours):

Advanced Internship Description (280 Hours):
Caregiving duties as seen above
Provide support to individuals based on Agape of Appleton, Inc. philosophy and values, ensuring care, safety and welfare of program participants
Support/advocate for individuals in community settings based on Agape philosophy and values
Develop and maintain therapeutic relationships with individuals in a variety of community settings
Support the Residential Case Manager with staff training, supervision and program oversight.
Effectively utilize individual direct service hours
Assert and safeguard the human and civil rights of individuals with developmental disabilities and foster the human dignity and personal worth of each individual
Provide assistance in case management/supports (inter-intra agency) in a professional manner
Assist in implementation of individual support plans/care plans
Provide staff training and supervision as directed by RCM, Agape of Appleton Inc. policies, procedures, and management philosophy
Be proficient and knowledgeable of all State/Federal regulations as specified by Agape of Appleton, Inc. policies and procedures
