Mission: Clarity Care is a non-profit organization that provides services for individuals with limitations or disabilities so they can achieve independent and fulfilling lives within their communities

Clientele: Children and adults with DD, PD, TBI, CMI, criminal offender

Services: We follow a four step process for assisting individuals with finding and retaining employment: 1) Discovery and planning stage 2) job development 3) training and support 4) follow up. These services are supported by our team of employment consultants, job coaches, and employment coordinator.

Internship Description (120 Hours):

• Learn basic case management skills

• Gain a basic understanding of the community employment process; begin to develop the language to advocate for a client in the community workplace

• Attend client staffings to gain knowledge and understanding of consultation and problem solving process to determine the best client care plan

• Shadow job coach coordinator and job coaches to gain understanding of the effectiveness and needs of job coaches in community employment

• Reach out to potential employers by sending letters and making follow up phone calls, expressing CES interest in working with the company and potential benefits to employer.

• Work with CES Director and Employment Consultants to complete departmental projects and future program planning

• Assist Employment Consultants with monthly DVR reporting elements and maintenance of client files.

Advanced Internship Description (280 Hours):

• Work collaboratively with Employment Consultant to manage case load

• Intake and follow along up to 5 clients with oversight from Employment Consultant

• Complete assessment and detailed interview to understand client’s needs and barriers in the workplace

• Utilize intervention and re-direction skills to assist client with maintaining focus and becoming community employment ready

• Assist client with understanding their barriers and determining a realistic work environment for their interest and skill set

• Learn to adapt personal teaching and assessment style to fit the needs of the current client and their unique needs

• Work collaboratively with DVR to determine the best employment goals within the client’s functioning capacity

• Attend and participate in client staffings to consult with colleagues to determine the best client care plan

• Provide direction to job coaches in regards to client needs when working in community employment

• Problem solve employer and job coach concerns for clients who are placed in community employment

• Reach out to potential employers via networking events to advocate for client’s interest in employer and potential benefits of community employment to employer
