The 5 P’s for a Perfect handover
When setting up another EP for a client. We want to set them up for success and also allow them to cater their appointment individually (rather than us writing a program, unless necessary)
Primary Concerns: diagnosis, clinically relevant information (dated), reason for visit
Purpose (goals): what is their goal or ideal outcome?
Prescription: what exercises/activities have you been prescribing? (eg. strength, gait, pain education)
Personality: 3 fun facts (eg. has 10 cats), likes/dislikes, things to be aware of (eg. don’t bring up their blood pressure)
Plan: What is the overarching plan for the handover (eg. you have 2 weeks with client x. Please keep working on balance with a particular focus on exercise X. The plan is to continue weekly)
