If there is an incident:
Staff members involved, or who witnessed the incident, must fill out the Incident Report Template
Form should be handed or emailed to staff member's manager who should co-sign on confirming the report
Manager should then email the form to ops@threesbrewing.com for follow-up and filing
What qualifies as an incident?
Any event that has led to a physical injury on the job (past examples include lacerations from an exploded growler; twisting an ankle while carrying beer)
Any physical altercation between guests, or between guests and staff
Any physical injury sustained by a customer at one of our locations (this would also include events like a customer fainting)
Any report of thievery (such as a customer or staff member reports a stolen item, taken from the premises, like a bike, a bag, etc.)
Any combative verbal altercation between staff members, staff & customer, customer & customer, etc.
Harassment of any kind
Other events not listed could also qualify. When in doubt, write it out.
