Employee Change Requests and Verifications of Employment Assistance

A Name Change request could be due to a recent marriage, divorce, or a court-ordered decision.  To update your name, you will need to submit some forms and documentation to HR. 

Please complete the following forms and submit them via email to DOR_OHR_JEC@state.co.us.

a.     Personnel Data Form (Save a copy to fill)

b.     PERA Membership Information Form (Save a copy to fill)

You will also need to provide a copy of a newly updated Social Security Card, as well as the following for your specific situation: Marriage - Registered Marriage Certificate, Divorce - Signed Divorce Decree, Court-Ordered Decision - Signed court documentation that confirms the official name change.

Have you recently moved and need to change your address? Follow one of these easy steps to make your address change:

1)     If you have access to the DOR Network via VPN Access, the best way to update your address is to access the Employee Self Service portal to update your information.

a.     Click on Change Residence Address & Phone Number to change your primary residence address and telephone number.

b. If you use a P.O. Box or alternative address, you will need to complete two steps:

(1) You can click on Change Alternate Address to view or change your Alternate Address. 

(2) You will need to inform HR that you have entered an Alternate Address in your record via email. This is to ensure your mailing address is marked correctly in your personnel record.

2)     If you do not have access to the DOR network, please complete both forms below by saving a copy to edit and submitting them to: DOR_OHR_JEC@state.co.us.

The US Department of Education has a Public Student Loan Forgiveness (PSLF) program. Additional information about the program can be found on the State's public page.  Click here to be redirected. If you already have the forms, please only complete the first page, and put your name and social security number at the top of the second page. HR will assist with completing the Employer Portion and getting the form back to you with instructions for submission.

When you need a Verification of Employment, the State has partnered with the Work Number.   The Work Number's Phone number is 800-996-7566 and they also have a website The Work Number.  The State of Colorado's employer code is 35892.
