New Badge Requests:
Supervisors that have new employees will need to submit a new DOR Badge Request Form. The supervisor is responsible for completing and verifying all information on the form.
Employee’s name on the badge form must be their legal name as it appears on the drivers license or State issued ID.
Once the form is completed and submitted, a photo of the employee that meets the photo criteria below must be emailed to the Facilities Access Request email listed on the DOR Badge Form.
Badge Photos must follow the below criteria:
Clear front image of the face
Both eyes and ears visible
Solid light-colored background
No glare (Remove glasses if it causes a glare)
No glamour or staged shots
No hats (Religious and Cultural garments and headwear may be permitted)
Photos must be JPEG format
Photo must have LAST NAME, FIRST NAME of employee
Once the badge has been processed, the employee and supervisor will be notified via email and provided details of its delivery.
Badge Replacement Requests:
If an employee’s photo ID Badge is lost or stolen, it will be replaced at no cost to the employee. If multiple badges are lost by the same employee, they may be charged for the additional replacement badges. Badges that are damaged or malfunctioning due to normal wear and use will be replaced at no cost, employees who intentionally damage or disable badges due to misuse or abuse, may be charged for replacement. Replacement badges for existing employees will require completing the Google Badge Form Request and selecting Replace Damaged or Lost Badge on the first page of the form.
All DOR employee ID badges expire 5 years from the date of original issue and activation. The access control system has a built-in security feature so that any ID Badge that is not used at least once in 180 consecutive days will automatically deactivate. These features are security measures to prevent badges from being lost or misplaced and used to enter DOR offices if found.
If your ID Badge has deactivated either due to non-use (not used at least once in the last 180 days) or it has reached its initial 5-year expiration period, all you need to do is send an email to dor_facilitiesaccessrequest@state.co.us and request reactivation. No badge paperwork is required.
Access Changes for Current Employees:
Access changes for existing employees require completing the Google Badge Form Request and selecting Access Change on the first page of the form.
If an employee is changing Divisions, a new badge form will be required as well as a new badge to reflect the new assignment.
Contractor Badges:
For any work that needs to be done to the DOR building, such as a remodeling project, where outside contractors or vendors need to be involved, contact Facilities Safety and Security at dor_facilitiesaccessrequest@state.co.us
Processing Time:
Generally, badge requests will be processed within two business days once all information is submitted and received by the Facilities Office.
