The SA-194 form is a records retention schedule which identifies the length of time a record series must be retained before its final disposition (destruction, preservation, or transfer). The SA-194 retention schedules are completed by each division’s Records Custodian or Delegate(s). The SA-194s are not approved until all required signatures are received at the bottom of page 1 (due to SB16-1368 enacted on 08/10/16 the Attorney General's signature is no longer required on the SA-194). Approved SA-194s are then posted on the State Archives website.
