The Colorado Department of Revenue’s only approved means by which to collect legally-binding digital signatures is via DocuSign. Your team can arrange to have this service enabled by reaching out directly to OIT. DocuSign’s services are not free and you will need to arrange for payment based on feedback from leadership and OIT pertaining to your specific signature-related needs.
Forms and Graphics Development can deploy basic, non-binding signature fields on any PDF file per your request. However, we have been advised by Policy to make our customers aware of the legal pitfalls of deploying non-authorized signature fields on public-facing documents. If you believe your document could potentially be needed in legal proceedings, we encourage you to explore DocuSign’s options or making the form unavailable for digital signatures all together.
In order to build out your Google Forms (if you choose to do it yourself), FGD has developed some basic templates for each division. The headers are prebuilt and placed and the colors are predefined per current DOR standards. There are two methods to generate your Google Form from the DOR-approved templates. To access these templates, follow the instructions below:
Method 1:
Make sure you're searching under State.co.us Executive Branch and not General (top left, under Template Gallery)
At this point you can scroll down to Revenue's section or you can directly jump to it using the "Jump to" link on the top right
Click the Division section that applies to you. Google will automatically create a copy of the template in your "My Drive" folder in Google Drive so you can find the source file at later time in that location. As this is a copy of the Template, you are free to begin building your form.

Method 2:
Navigate to Google Drive
Select "NEW" from the top left corner
If "Google Forms" isn't visible from your initial palette/window, navigate down to "More"
Instead of directly clicking on "Google Forms", click the "arrow" to the right of the text to open up further options
At this point you'll be prompted with two options, choose "From a Template"
Now the Google Templates page will open
Make sure you're searching under State.co.us Executive Branch and not General (top left, under Template Gallery)
At this point you can scroll down to Revenue's section or you can directly jump to it using the "Jump to" link on the top right
Click the Division section that applies to you. Google will automatically create a copy of the template in your "My Drive" folder in Google Drive so you can find the source file at later time in in that location. As this is a copy of the Template, you are free to begin building your form.

With the move from Microsoft Office products (including Word), Forms and Graphics Development will no longer provide .DOC or .DOCX files as letterhead options. It's still possible for you to create these versions on your end by using the DOR-provided Google Doc version of the letterheads and navigating to the File > Download > .doc from the browser menu bar. Use the instructions below to find the letterhead for your section.

Some of the files are automatically hidden by Google (for brevity), you may need to click the arrow underneath the Revenue letterhead options to open up additional DOR templates.
Click the Division section that applies to you. Google will automatically create a copy of the template in your "My Drive" folder in Google Drive so you can find the source file at a later time in in that location. As this is a copy of the Template, you are free to begin working on your document.

The CDOR PowerPoint presentation template is available for download and use throughout the Department. The PowerPoint template has the DOR approved colors on the first slide for your use. Fonts should be Trebuchet (header) and Arial (body). The Google Slide version has the colors pre-loaded under colors>custom. Fonts are pre-set as well.
