Forms and Graphics Development
Responsible for the physical security of DOR facilities, assets and access for employees, contractors and other State employees doing business at our facilities. We manage employees’ access from when they are hired until they leave employment through the Exiting process, as well as keys for offices and suites, and video surveillance at many offices across the state.
(All badge requests MUST be completed by supervisor or manager only)
Employees and contractors are required to wear a State issued Department ID whenever working on DOR premises. DOR ID Badges help to identify employees (full-time, part-time and temporary) and contractors from the regular public and individuals that are not authorized to be in buildings. IDs should be worn on the outermost layer of clothing or plainly visible to others. DOR ID badges are also used to provide access to DOR buildings with card readers.
There are different types of DOR ID badge classifications.
State employee - For full or part-time employees
Temporary to DOR - Seasonal or hired through Staffing agency
Police - State employees sworn as law enforcement officers
Contractor - For Facility Safety & Security use only
Visitor- Department ID only, no access levels are assigned. Holder must be escorted. Intended for public that is brought into secured areas
Supervisors submitting the request must certify on the ID Badge Request Form that background checks have been completed by OHR.
To request a DOR ID badge, supervisors must complete the DOR Badge Request Form.
If an employee’s photo ID Badge is lost or stolen, it will be replaced at no cost to the employee. If multiple badges are lost by the same employee, they may be charged for the additional replacement badges. Badges that are damaged or malfunctioning due to normal wear and use will be replaced at no cost, employees who intentionally damage or disable badges due to misuse or abuse, may be charged for replacement.
Lost or stolen badges must be immediately reported to the Office of Safety & Security and the Program Manager or Division Leader. Failure to promptly report a lost or stolen badge may result in unauthorized use and the individual employee may be held accountable.
Visitor Badges do not have any access levels or permissions and are for single-visit use only. All persons issued a visitor badge must be escorted by an employee whenever in an area that is not open to the public.
Visitor Badges are for non-employees, contractors, vendors, or any other person that has not been background checked and is not authorized to be in employee or restricted and secured areas.
Please review the update process before requesting badges.
DOR Day Passes are made specific to a program within a Division, we have 2 different types of Day Passes: Standard Day Pass and Building Services.
Standard Day Pass - These are intended for employees from other offices/divisions that do not require to be escorted, employee’s that forgot their badge at home or a new employee that has not received their permanent badge. Day passes are not to be used as a replacement for a lost badge. The Day Pass provides basic access Mon-Fri 8:00 am to 5:00 pm for the Building, Division and Program displayed on the badge.
Building Services - These are issued to companies that provide building services to DOR properties such as landlords, property managers, janitorial staff and maintenance personnel.
Access levels are determined by the employee’s Supervisor or Manager. Access requests to a building or rooms that belong to another Division or Program will require approval from that Division’s appointing authority. “Badge will ONLY have access levels noted on this form (all previous levels will be removed)”
To request an access change to an existing DOR badge, supervisors must complete the DOR Badge Request Form and select Access Change on the first page.
