This page provides information on how to obtain badge access and requests for additional resources.
To request a new DOR ID Badge or to make changes to an existing employee badge, the employee's supervisor or manager must complete the DOR Badge Request Form. Contact the Facilities Office if you have furniture, fixtures or equipment needs that can not be addressed within your Division by using the Howard Service Request Form. Envoy is the visitor management system that staff members need to use to receive notifications when someone arrives on-site for them. Employees who are assigned to the Howard Building and will have visitors, will need to set this up on their work computer and/or mobile device to be notified when visitors arrive. Please note that the front desk will not be providing any notifications.
Keys for offices on the 4th Floor of the Howard Building are managed and issued by CDOT. Requests for Howard Building keys should be submitted to FSS using the Howard Building Key Request Form below. All request forms must be electronically signed by both the employee and their supervisor. Completed key request forms must be submitted to the Facilities Access Request email dor_facilitiesaccessrequest@state.co.us to be processed.
Please allow 3 to 5 business days for processing.
Employees will be notified when the key has been assigned and is available for pickup at the Facilities Office, Room 421.
Requests for furniture, desk and cabinet keys should be submitted to the Facilities Access Request email. (No key agreement required)
