All requests for new and replacement badges or access changes to existing badges must be submitted through the Google Badge Request Form found on the FSS Intranet page.

Required fields are marked with and asterisk and must be completed in order to proceed to the next section.
Access Levels
Check all authorized levels in each section, restricted and secure areas will require approval of appointing authority for that section if it’s not yours.
Only one building or location can be selected at a time, access for multiple buildings will need to be selected one at a time.
(ALL ACCESS LEVELS FROM PREVIOUS FORMS WILL BE REMOVED UNLESS SELECTED ON THE NEW FORM)
Photos are not required for replacement badges or access changes only.
Photo criteria:
Photos must be emailed Facilities Access after the form is submitted.
Clear front image of the face
Both eyes and ears visible
Solid light-colored background
No glare (remove glasses if it causes glare)
No glamour or staged shots
No hats (Religious and Cultural garments and headwear may be permitted)
Photo must be JPEG format
Photo must have LAST NAME, FIRST NAME
It is the supervisor’s responsibility that ALL information and photo submitted is correct and verified for authenticity of the person listed on the form.
Processing Requests
Badge requests will be processed in 24-48 hours in most cases depending on workload.
Walk-in requests are not available due to other obligations and scheduling of the FSS staff.
You will be contacted if there are any questions.
The employee and supervisor will be notified via email when the badge is completed.
If badge is being mailed, activation instructions will be provided.
