This Google Form (DR 4691GF) is to request a project be updated or a new project created by Forms & Graphics Development. It helps guide you on submitting a new request or updating projects. Whether you need updates to forms, covers, brochures, documents and more. This request is for new projects as well. Just click and follow the instructions. This replaces the DR 4691 Project Request.
This option is now included in the DR 4691GF; the DR 4691R has been obsoleted.
The Google Form is used to send a request to Forms & Graphics Development to create a Review PDF for Document/Form Owners, Content Managers and Subject Matter Experts to review. This is for projects that have DR numbers or that Forms & Graphics Development has previously worked on. It is preferred that a review PDF is requested so that FGD is able to better track changes made to projects.
This option is now included in the DR 4691GF; the DR 4691P has been obsoleted.
The Google form is used to request that FGD create a new fillable PDF. This request is only used when a project or form has never had a fillable PDF created for the project and a fillable PDF is now needed. This is not needed if the requestor has requested a project be updated or a new project created using the "Project Request" since the fillable PDF is covered in the "Project Request".

For experienced staff, please fill out this form electronically and email it to DOR_FD_Print@state.co.us for prints or quick copies that go to IDS. If this is for a job that has previously been processed by FGD, please email us and we can resurface the old print request to save you time. This Print Request is for forms and any other project that Forms & Graphics Development updates that need printed through IDS. We can work with you to print products developed outside of Forms and Graphics Development as well.

Waivers are requested through IDS. Waivers are needed for projects that are not sent through IDS for printing. The Form Coordinator or Forms & Graphics Development need to fill out the waiver after a quote is received from the vendor who will be printing the project. This obsolete project request is to be used when a Form Coordinator or Content Manager has decided to obsolete a project that is no longer in use. The request will be shared with other sections. This is to ensure that the form or project is not obsolete if it is used by other sections or divisions.
After 30 days from the receipt of request the project files, hard copy and electronic copy, will be deleted and destroyed. The DR number will be added back to the list of numbers to be assigned.
If the form or project was used by the public the project will be considered a Master Form or Document. In this instance the approved versions of the document will be placed an a Master folder and the DR # will not be reassigned. While it is technically obsolete and no longer used the Master file will be held indefinitely.
The Name Plate Request (DR 5999GF) is a Google Form.
One standard style Name Plate is provided free to CDOR employees. Font, color and size used are standardized and will not be changed. Name plate requests outside the standard style and design will NOT be provided by Forms Development (FD).

The Google Form will be used to request a name tag be created for use. Font, color and size are standardized and will not be changes. Name tag requests outside the standard style and design will NOT be provided by FD. A name tag may be requested by the Supervisor or Manager, or an Executive Administrative Assistant. Name, Title and reason for the name tag will be requested.
