X-Linked Adrenal Leukodystrophy
X-ALD
OMIM:300100
Gene:ABCD1
Chromosome:X28q
Protein:
Acetyl CoA oxidase deficiency
Peroxisome Membrane protein
Needed for transport of very long fatty chains, so they can be processed in the peroxisome to shorter bits that can be used to create more complicated lipids and acetylation of proteins
Accumulation:
Very long fatty chains
Leads to demyelination in white matter and spinal chord
Test:
Very long fatty acids in plasma, red blood cells, fibroblasts
Chomosomal studies (preferred method in women)
Childhood cerebral type
Age:5-12 years
Sex:Male
Clinical presentation:
"ADHD"
Visiospatial disturbance
Gait
Possible spinal or periferal symptoms
Slight intelectual impairment
Abnormal pigmentation sometimes
10% acute:
Siezures
Adrenal crisis
Coma
Eventually:
Hypotonia
Siezures
Swallowing difficulties
Imaging
80%

Pathology of main type
Zone A:Burned out
Zone B:Perivascular demyelination, preserved axons
Zone C:Myelin break down without demyelination
(Zone D): Impending? at least low NAA
Adrenomyeloneuropathy
Age: Mean 28
Sex: Male
Clinical presentation:
Incontinence
Progressive paraparesis
Cerebellar dysfunction
Can be combined with cerebellar involvement
Imaging:
Cerebellar white matter
Corticospinal tracts
No enhancement
Female heterozygous
Age:Mean 43
Sex: Female
Clinical presentation:
Like adrenomyeloneuropathy but milder
Imaging:
Usually normal
MRS as compared to normal controls in corticospinal and pareitooccipital
Reduced NAA/Choline
Reduced NAA/Cr
