Upper Midbrain

RN - red nucleus

• 1. Fibers of the rubro-olivary tract project from the parvocellular red nucleus via the central tegmental tract to the ipsilateral inferior olivary nucleus.

  2. The inferior olivary nucleus sends its afferents via climbing fibers in the inferior cerebellar peduncle to Purkinje cells of the contralateral cerebellar cortex.

  3. The Purkinje cells send their afferents to the ipsilateral dentate nucleus.

  4. The dentate nucleus afferents travel via the superior cerebellar peduncle to the contralateral red nucleus, thus completing the cycle.

Pyt- Pyramidal tract= 

corticospinal

(broddman4(30%)+premotor+somatosensory [...]->alpha motor neurons) 

90% crossses=lateral corticospinal:extremies

10% does not cross=medial corticospinal:trunk

corticobulbar(broddman4->lower motor neurons of non occulomotor cranial nerves)

Bilateral innervation of cranial nerves except facial below the eye and XII(hypoglossus)

SN Substantia Nigra 

atrophy (disputed) in parkinsson and MSA - pars compacta is affected.

ML Medial Lemniscus

Somatosensorisk information (vibration and touch ?proprioception) - kopplas först till nucleus gracilis och cuneatus.

CNIII - Occulomotorius

Framför periaquductal grey

består av flera kärnor bla Erdinger Westphal

fibrer sträcker sig genom eller medialt om nucleus ruber, 

kommer ut mellan a cerebri post och a cerebelli sup 

överst av kranialnerver i sinus cavernosus och lateralt om ICA genom superior orbital fissure- övre del innerverar palpebrae och nedre del

medial+inferior rectus, inferior oblique och sphincter pupilla.

SCP Superior Cerebellar Decussation

Decussation of fibres from the dentate nucleus via the superior cerebellar peduncles -> nucleus ruber

CN IV trochlearis

framför periacueductal grey

fibrer korsar bakom akvedukten och ut bakåt/lateralt och framåt i cisterna ambiens

innerverar superior oblique - ger vertikal och torsional diplopia

MLF medial longitudinal fasciculus

kopplar ihop ögonmotoriknuclei 

CN III occulomotorius, CN IV trochlearis, CN VI abducens

koordinerar saccades, smooth persuit och vestibuloccular responses.

kopplingar med vestibulära kärnor koordinerar ögonrörelser med huvudrörelser.

Internucleär opthalmoplegi kan uppstå vid skada här vid MS eller stroke - 

Upper pons

- level of superior cerebellar peduncles

Basis pontis - TPF och framåt. Bakom detta är det tegmentum.

TPF Transverse Pontine Fibres

Flera pontina nuclei finns här, fibrer från frontal, temporal och parietallober går genom här 

och formar nedanför de mediala cerebellära pedunklarna

RF Reticular formation

Lateral/Medial/Median 

LL - lateral lemniscus

fibrer från cochlear nucleus till andra nuclei och inferior colliculus

CTT - Central Tegmental Tract

Connecting nucleus ruber with the inferior olivary nucleus - en av de första som myeliniseras - känslig för asphyxi    

Består av pallido-olivary, rubro-olivary, reticulo-olivary fibres (part of Guillan and Mollaret)

STT - spinothalamic tract

sensoriska banor från hud till thalamus.

CN VII,VIII går ut på denna nivå.
