The rule states that an A/C must have enough speed for the next highest turn rate if executing an NG transition. Does this rule apply for EZ transitions too, ie the a/c must have sufficient speed to fulfill a TT rate, or does the general "EZ turns can always be executed" rule (5.2 Turning Procedure) apply?
I would say that it must meet the TT speed in order to do a Neg-G EZ, as this is a "special" condition required to do the Neg-G turn.
The "Ez turns can always be executed" applies to Pos-G turns.

An aircraft in an upright bank makes a Neg-G turn. The aircraft is in SHD.
The Rev 3 Flight Rules (v3.4-3.51 at any rate) say:
Upright A/C completing NG turns must either adjust their flight att. down by one step toward earth at end of their move or lose 100 feet per NG facing change done. This transition is at no decel cost and the VFPs used exception of Rule 6.1.4 is negated for the next turn.
The Draft 3.4 rules don't tell much more:
NG Turning Forced Transition or Altitude Loss: Completing one or more NG turns in a game turn from an UPRIGHT bank angle requires the A/C to either lose 100 feet (one increment) per 30° of NG facing change completed (normal dive accel applies), or to adjust the A/C's ending flight attitude one step in a nose down direction, free of cost, at the end of its move, before combat.
Both of these passages imply that the 100 ft (-.1) altitude loss is extra, beyond normal flight.

If the aircraft was flying level, clearly a -0.1 OC would be required (if selected instead of dropping the nose).

It is NOT included in the OC. This is a special case and the loss would be noted on the FP that the NG Turn was completed.
Similar to the way the optional -0.1 loss is recorded for a Slip in the direction of the bank.

Equally, how would we notate an OC of +.1 used to counter a drop of .1 due to a negG turn?
The OC of +0.x can only occur if the ENDING attitude is Level or Climb. If the +0.x OC is valid, then use it. OC is independent of NG Turns.
Finally, you can NOT combine BOTH NG Turn penalties in a single move. You must pick one for that move. However, this does not prevent you from using the -0.1 on one turn and "Drop the nose" on the following turn.
You can NOT do BOTH if you complete two NG Turns in a single move.

I'm currently facing a situation where a player has carried an NG turn into this turn (T3). At the start of the turn (T3), he abandons the NG turn and proceeds with positive G turning. Should he be at forced idle next turn (T4)? The rules don't specify.
NG effects are in play, since the a/c was -G at the start of the turn.
From 3rd Ed rules -
"5.9 NEGATIVE-G TURNING
"NG Turns and Combat: Aiming for ground attacks (Rule 18.1), tracking shots, long bursts and defensive fire (Chapter 10) are not allowed if NG rates involved in any manner during a game turn (this includes having NG turn carry). "

"4.4 MAX & MIN SPEED EFFECTS
"Minimum Speed Effects: If an A/C's start speed is less than its min adjusted speed, it stalls. If an A/C initiates a turn or pitch transition with insufficient speed, or starts a game turn with a speed less than that required for a carried turn rate, it suffers a turn stall (4.6). A/C making a gun attack with an end speed less than minimum adjusted speed have a +15 LH shot modifier. "
and
"4.6 TURNING STALLS
"• When A/C carry a turn forward (Ch. 5) and then have a start speed insufficient for the carried turn rate."
and
"Slip & Skid Combat Effects: If in a slip or skid (FPs carried), or if the last FP before firing was used to execute either, or if firing after doing a skid at any point in the same game turn, LH modifiers apply to the shot. "

4.3.1 –– LIMITS ON SPEED & POWER
Non-Fuel Injected Engines: A/C are deemed to have positive pressure fuel injected engines unless "No F.I." appears in the engine description of their ADC. "No F.I." A/C will be forced to use idle power in the following situations:
1. Inverted Flight Forced Idle: "No F.I." A/C that spend an
entire game turn in an inverted attitude, and which do not
also use either a pull transition or do positive-G turning,
must use idle power on the following game turn.
2. Negative-G (NG) Forced Idle: "No F.I." A/C that do NG
transitions (rule 6.5), start NG turns (rule 5.9), or carry NG
turning forward must use idle on the following game turn.
