The "Oral History Project Final Requirement" handout below includes requirements for the paper (page 1) and the oral presentation + the schedule (page 2).

Release forms are uploaded below. The single page sheet is for interviewees who do not want their names used in the paper (pseudonym forms). The 2 forms to a page form is the regular release form. Every interviewee must complete one of these forms.
