An excerpt from George Lucas: The Creative Impulse:

“The film’s dazzling visual effects include a Broadway theater that never was (inside or out), street scenes to be found on no known map, and 6 dancers transformed into choruses of 30, 60, and finally 120, for loopy and lavish production numbers in the spirit of 1920s Broadway.”

When they say "street scenes to be found on no known map," they are certainly right - most of the city street shots were filmed on the former backlot set at Screen Gems Studios in Wilmington. Sadly, this set was torn down and replaced with a soundstage. Click here to read the full story.

A nearly identical shot in "Biloxi Blues"

The railway used for the opening shots was the New Georgia Railroad, an excursion train that did an 18-mile loop around Atlanta in the 1980s. The engine is the Savannah & Atlanta #750, built in 1910 (and also featured in Biloxi Blues). This engine is now on permanent display at the Southeastern Railway Museum in Duluth, Georgia.

The North Carolina Court of Appeals Building plays the part of Grand Central Station where Peggy & Indy arrive to the city. Side note - the towering building digitally added behind it is the Russ Building in San Francisco's Financial District.

The "Horseshoe Lounge" was located on the backlot set at Screen Gems, in the same location where "The Royal Garden Club" stood in Mystery of the Blues.

The "Victoria Hotel for Women" was the Market Street Apartments building at 420 Market Street in Wilmington. This building was also featured in Blue Velvet.

Park scenes were filmed at Airlie Gardens in North Carolina, also used in Princeton 1919. The buildings were added with a matte painting.

The "Chez Maurice" is Caprice Bistro at 10 Market Street in Wilmington.

This is the Federal Courthouse in Wilmington. Indy enters the party through the door on the NW side of the building, near the intersection of Princess Street & North Water Street. This same building was seen in Mystery of the Blues.

Indy & George are later seen back in front of the NC Court of Appeals Building at 1 West Morgan Street in Raleigh, NC. 

The street that Kate's apartment is on was actually the backlot set at Screen Gems.

Interior shots of the theater were filmed at Thalian Hall in Wilmington.

However, during the final performance of the show a few shots are mixed in that are obviously the interior of the National Theatre in Prague.

The shots of the outside of the theater were a matte-painted version of the front steps of the Rudolfinum Concert Hall in Prague. This location was used several times throughout the show.

The hotdog stand where Indy & Peggy grab a quick bite was a part of the backlot set at Screen Gems.

The "Hotel Algonquin" is the Sir Walter Raleigh Hotel, the oldest hotel in Raleigh, NC.
