It is best to save it to your drive as a TEMPLATE and then make copies of it whenever you need to order something. Remember to take off the example supplies in the table.
Quantity: how many you want
Unit: how many is included in one package/box (i.e. 1000 tubes or 100 mL)
Catalog No.: For Fisher Scientific, it will be labeled as the catalog number. For Amazon, it will be the ASIN number (you will have to scroll down to Product Details for that information).
Description: the name of whatever you are purchasing (with enough information to not have it mixed up with another product)
Unit Cost: the price of one (not the total price, the price if the quantity = 1).
When finished and you have double checked everything, email it to Dr. Norimatsu for approval and cC Leah Keller (our secretary; you can find her email on the ATSU website or ask a lab leader; if you cannot cC her, Dr. Norimatsu will send it to her himself after approving the form).
