The purpose of this document is to establish the policy of Flogistix requiring pipe plugs to be in any pressure valve that can be opened to atmosphere. The purpose of this policy is for any valve that may be used to vent or check the pressure where the valve is located to have a pipe plug installed in the valve when not in use. By having a plug in the valve it prevents the accidental release of pressure from that valve. This policy is for field applications or at service facilities.
This Policy is in effect for all Flogistix service locations, field locations, or well sites where valves are used on pressure vessels, gas or liquid piping when used to check pressure or to drain fluids.
Valves without plugs installed could release high pressure gas or fluids, toxic vapors, or liquids that could cause environmental damage to people, animals, plants or soil and could lead to death. Gas vapors could add to the potential cause of a fire resulting in property damage to the compressor unit, surrounding equipment, or facility.
Allowing Flogistix equipment to operate with pressure valves that can be opened to atmosphere without pipe plugs installed is Prohibited.
All pipe plugs used in pressure valves, piping, or vessels need to be forged steel plugs rated for high pressure applications. The use of cast iron plugs is Not Allowed due to the low pressure rating of the cast iron plugs
Management - Management is responsible for implementing, supporting, and enforcing the requirements of this procedure to their respective locations/area.
EHSS - Responsible to assist in the implementation of this procedure and to review for effectiveness.
Employees, Contractors, Vendors, ETC. - Are responsible to follow the follow the requirements outlined in this procedure and ask for assistance
This policy applies to the use pipe plugs in all pressure valves that can be opened to atmosphere at any Flogistix location including field locations, manufacturing, and well sites that are currently being maintained, or being contracted to work on by Flogistix personnel. This policy is applicable to all Flogistix employees, contractors, and temporary employees.
Employees installing pipe plugs on pressure vessels or pressure piping need to be aware of the possibility of Hazardous gas. Employees should be aware that units could contain flammable and toxic gasses. Potential for the possible hazards of gas ignition or fire, hot surfaces & fluids. Risk of possible concentration of H2S. Hazardous gases could be in the units pressure vessels, cooler, or associated piping and risk of inhalation of hazardous gas fumes.

Four Gas Monitor, if nipple is being replaced in the field which is outlined in the Flogistix EHSS Manual, Section 8, Procedure 8.3 Personal Protective Equipment (PPE)
This procedure applies to all units in service in the field, at a service shop, or in any storage yard.
Insure Flogistix personnel have proper safety certification or site orientation if required by the Flogistix Customer.
Insure Flogistix personnel adhere to any Customers safety practices.
All employees shall be initially trained on the following topics:
Employee Personal Protective Equipment (PPE)
Lock Out Tag Out (LOTO) procedure according to Flogistix policy
Emergency and First Aid procedures
Methods and Observations that may be used to detect the presence or release of a hazardous chemicals or gas in the work place
Tools & Equipment Required
All employees performing the installation of pipe plugs in pressure valves shall be required to have the listed tools to perform job task:
Wrench capable of tightening the size of the head of the pipe being installed
