1. OPTION 1 - INSURANCE: Parents may purchase insurance to cover damage of the device. The technology department has made arrangements with a private insurance carrier to offer this option to students and parents. If this option is elected:
The appropriate paperwork must be completed by the parent(s) to enroll in the insurance plan.
The school should collect the insurance fee and deposit it into the school's appropriate program account.
Periodically, the school should issue a check to the private insurance carrier with the appropriate information needed to enroll each student in the insurance plan.
IMPORTANT: It is critical the school make sure procedures are in place to remit funds and plan enrollment to the private carrier on a timely basis. Otherwise, the student will not have coverage.
2. OPTION 2 - ASSUME LIABILITY: Parents may assume full liability for damaged equipment. If parents elect this option, they must sign a form indicating their intent to fully pay for all damaged equipment.
