Instructions for Parents: Online Free/Reduced Lunch Applications for Parents

Dear Parents, August 1, 2013

As we prepare for the upcoming school year, we would like to welcome all students to participate in the school breakfast and lunch programs. The following are the rates for the

2013-2014 school year.

Breakfast Lunch

Elementary $1.30 $1.95

Secondary $1.40 $2.35

Reduced All Grades $ .30 $ .40

The district would like to encourage ALL families to apply for the School Free and Reduced Lunch Program for the 2013-2014 school year. This program allows for qualifying students to receive free or reduced price breakfast and lunch throughout the school year. Applicants need to apply on a yearly basis. This program is very beneficial not only to families and students in our community, but also to our overall Food Service Program. All information submitted is confidential and the students are not segregated or separated in any way.

This year we have online lunch applications for your convenience. Please follow the instructions listed below. If you have any additional questions, please feel free to contact Bonnie Bairett at 435-438-2291, ext. 6001. Please remember to complete and submit the application before school begins. Again, we encourage each family to apply. (Please note: The Fee Waiver Program is separate and is handled at the school level. Contact your school for more details.)

Online Lunch Applications:

If you have already created a Username and Password, Login and follow directions from * forward.

                  • Your Students ID Numbers

  • Under the Footprints Choose “CLICK HERE for

access to SIS to set up your Parental Account”

From here you can Login or Create a User Account

  • Select “Request a User Name” (Under the Blue Box)

  • Enter your child’s Student ID and Date of Birth.

§ Confirm Student

§ Choose Contact

§ Create Username and Password

§ Select “Create Account”

Select “Free/Reduced Applications”

At Bottom of Page Select “Apply Now…”

Choose “Apply in English” or “Apply in Spanish”

Select “Begin Application”

Be sure to fill in all areas completely. A confirmation page will show that your

Application has been correctly submitted.

v Remember the school district has 10 days to approve your application.

v You will be notified by phone message if you have been approved. If you are denied you will be notified through a letter in the mail.

Ø If you do not have access to a computer, the computer lab will be open at the schools during registration and back to school night.
