Splenic Injury - classification
Splenic injury Classification:
--grade I: CT scan shows subcapsular hematoma of < 10% of spleen surface area, or having a lac. of < 1 cm deep.
--grade II: CT scan shows subcapsular hematoma of 10 - 50% of spleen surface area, or a parenchymal hematoma of < 5 cm in diameter or lac 1 - 3 cm deep, not involving trabecular vessels.
--grade III: CT scan shows subcapsular hematoma > 50% of spleen surface area, or a parenchymal hematoma of > 5 cm diameter with no expansion or rupture.
--grade IV: Lac. of segmental vessles with devascularization of < 50% of spleen
--grade V: "shattered spleen," devascularization of > 50% spleen.
grade I - III are usually hemodynamically stable and required repeat CT and US exams.
grade IV and V required splenectomy.
