  • • Home: relationship with parents, family, siblings. General home life

    • Educations: school performance, grades, problems.

    • Activities: hobbies, after-school activities, interests, sports, music preferences, friends, and involvement in gangs. How many hrs spent watching TV, listening to radio, video games, on phone, computer games.

    • Drugs and Diet: tobacco, ETOH, drugs, diet.

    • Sexuality: relationship, sexually active, men, women or both, partners. STIs, contraceptive method.

    • Suicide: ideation or intent, depression.

    • Safety: seatbelts, helmets, guns. Violence, accidents.

• • OB&GYN: menarache, LMP, freq, duration, flow, dysmenorrhea. Last Pap test? STIs. Pregnancies..
