Falls
Ask the nurse for a bedside glucose test and vital signs.
Go see the patient.
Find out from the patient what occurred and where the patient is having pain. Also talk with witnesses.
Perform a neurological exam, including LOC and head examination and look for hematomas.
If you have any concerns about head trauma or note a change in mental status, check a stat head CT without contrast to rule out a bleed.
Check recent lab results and ECG/Telemetry. Consider checking for possible trauma.
Look at the medication list for possible contributing factors (sedatives, anti-HTN, hypoglycemia agents).
Clearly document what occurred in the patient's chart.
