Falls

    1. Ask the nurse for a bedside glucose test and vital signs.

    2. Go see the patient.

    3. Find out from the patient what occurred and where the patient is having pain. Also talk with witnesses.

    4. Perform a neurological exam, including LOC and head examination and look for hematomas.

    5. If you have any concerns about head trauma or note a change in mental status, check a stat head CT without contrast to rule out a bleed.

    6. Check recent lab results and ECG/Telemetry. Consider checking for possible trauma.

    7. Look at the medication list for possible contributing factors (sedatives, anti-HTN, hypoglycemia agents).

    8. Clearly document what occurred in the patient's chart.
