ECG RAE/LAE

LAE:

    • Wide >2.5 mm (100 msec) in lead II

    • P waves are biphasic in leads V1-2. If the second, negative phase of the P wave is >1.5 mm in depth, it indicates LAE.

    • In mitral valve disease: wide "double-humped" P wave >120 msec duration in lead II.

RAE:

    • P waves amplitude >2.5 or more in leads II, III

    • Peaked P wave morphology (P- pulmonale). Duration >10 msec

    • Sharply peaked biphasic P wave in lead V1 - V2 with initial deflection greater (>1.5) than the terminal deflection

Biatrial enlargement:

    • Combination of tall P waves in II, III, aVF with terminal negative V1.
