HFPEF, Diastolic Dysfunction

Etiology of Heart Failure with Preserved Ejection Fraction (formerly known as Diastolic dysfunction):

    • Epicardial ischemia - UA, MI

    • Hypertrophic states - HTN, LVH, AS, IHSS

    • CM - HCM, DCM

    • Decreased diastolic filling - AF, Sinus tachycardia

    • Severe MS

    • Aging

    • Hypothyroidism

    • Increased passive stiffness

    • amyloidosis

    • hemochromatosis

    • Post MI - endomyocardial fibrotic scarring, aneurysm

    • Pericardial constriction

    • Pericardial tamponade

    • Right ventricular overload

    • Fibroelastosis

    • Compression by tumor

    • Beta blockers.

Diuretics and nitrates must be used w/ caution.
