Risk assessments
HSM166 is the master risk assessment template and includes the risk criteria to use.
Risk assessments should be conducted by a group with a variety of disciplines and knowledge.
Risk assessments should have a lead who can provide structure for methodically covering the different areas under assessment.
Additional controls should be captured on basecamp with deadlines to track implementation.
Accident & near Miss Reporting
We must keep a record internally of all accidents, incidents and near-misses. This is a both a legal requirement and best practice to support learning on how we can mitigate future accidents.
This is achieved by staff training training as described by HSM018 and reporting through - Accident and Dangerous Occurrence Reporting Form
The findings from notable incident investigations will be fed back to staff.
RIDDOR Reporting
The RIDDOR regulations dictate that more serious incidents must be reported to HSE.

Who should report?
Only 'responsible persons' should submit reports under RIDDOR. For Ballard UK/Arocla, this is Ben Todd.
Reporting online
Responsible persons should complete the appropriate online report form listed below. The form will then be submitted directly to the RIDDOR database. We MUST download a copy for our records and file with our internal Reporting records.
